Authorization

I authorize United Quality Collision to disassemble my vehicle for the purpose of diagnosing repairs and preparing a complete estimate. Once approvals are received from my insurance company, I authorize United Quality Collision to order all parts needed to complete the repairs. My vehicle may be driven around the facility for testing and also to any sublet vendor in order to complete repairs.

Direction to Pay

I authorize the insurance company to issue payment directly to United Quality Collision for any repairs or supplemental costs related to this claim. I understand all payments, confirmations, and supplements must be received before releasing the vehicle. If payment is sent to me in error, I agree to bring it to United Quality Collision. Debit card payments have no additional cost, and credit card payments may include a 3% processing fee.
